Create a Pathway Plan

The plan will set out your needs and wishes and how best they can be met by yourself and the County Council. It will be flexible – it’ll develop and adapt as your life changes. It covers the time from when you leave care, to when you reach your early 20s. The idea of the Plan is to make sure you know your rights, get access to the information you need to make choices, and get support to enable you to pursue your hopes and ambitions. The bottom line is - remember it’s your future.   These are just a few examples of the sort of practical help your Pathway Plan could get you:
  • rent and living expenses
  • money for sports or hobby equipment
  • fees for education and training
  • finding accommodation in the holidays (if you are at college)
  • careers and job information / preparation

The After-Care Service is a long term commitment to give you a good start in your adult life. If you decide to leave Anglesey,  your After-Care Worker will keep in touch and make sure you get continued support from your new Local Authority.

You are entitled to submit an official complaint to the Department if you are unhappy with our services.
