Personal licence to sell alcohol


Every sale of alcohol from a licensed premises has to be authorised by a Designated Premises Supervisor who is the holder of a personal licence.

You must be 18 years or older and hold a licensing qualification

Apply for a personal licence

Please complete the online form and pay the relevant fee.

Once you have completed the online form please submit the following accompanying documents to complete your application.

  • Completed application form.
  • A basic DBS disclosure form issued no earlier than one calendar month before you submit this application. (The checklist on the application form lists other acceptable documents).
  • Relevant licensing qualification.
  • Proof of right to work in the United Kingdom.
  • Two photographs (see checklist on application form).

You may wish to send your documents by recorded delivery.

Change of personal licence holder’s name and /or address

Please complete the online form and pay the relevant fee, if you would like to change the name on your personal licence, or if you have changed address.

Please note that you must apply to the original issuing Authority.

You will also need to complete and submit (upload) a notice of change of personal licence holder’s name and/or address to complete your application. 

Theft, loss etc. of personal licence  

Please complete the online form and pay the relevant fee if you would like a replacement personal licence.

Please note that you must apply to the original issuing Authority.

You will also need to complete and submit (upload) a notice of theft, loss etc. of personal licence to complete your application.
