Next Steps Transition Service
The Next Steps Transition Service is a multi-agency service for young people with developmental delay or disorder, disability, or illness who require a co-ordinated service from Children’s Services through to Adult Services from the Health Board, Social Services and the Education Department on Anglesey.

Information about your first Transition Review.
What happens when you are 16?
After a review, we will complete a Transition Plan about what will happen when you leave school. You will be given a copy of the Transition Plan.
