Becoming an adult
Leaving school and then moving into adulthood can be a challenge for young people and their parents, as they are unclear of what kind of choices exist, and how will they help the young person lead a fulfilling life.
Next Steps Transition Service
The Next Steps Transition Service is a multi-agency service for young people with developmental delay or disorder, disability, or illness who require a co-ordinated service from Children’s Services through to Adult Services from the Health Board, Social Services and the Education Department on Anglesey.
