Serious Case Reviews (SCR)

It is the board’s duty to undertake a “Serious Case Review” (SCR) where abuse or neglect of a child is known or suspected and:

  • a child dies; or
  • a child sustains a potentially life-threatening injury or serious and permanent impairment of health or development, this may include cases where a child has been subjected to particularly serious sexual abuse. 

The Local Safeguarding Children Board (LSCB) for the area must conduct a Serious Case Review.

Additionally, LSCBs should always undertake a serious case review where:

  • a child has committed suicide; or
  • the child has been killed by a parent with a mental illness

A board may also undertake a serious case review where a child within its area suffers harm that does not meet the criteria set out above but where there may be concerns about, for example:

  • inter-agency working; or
  • local procedures or policies

Historical Serious Case Reviews (SCR’s) are withdrawn from the  LSCB’s website upon confirmation by Designated Agency Senior Managers that progress has been achieved against recommendations contained in each SCR. Individuals seeking access to Executive Summary’s of historical SCR’s completed in the Gwynedd and Anglesey area or seeking further information should contact the LSCB Business Manager (01286 679 926).
