If we provide services to meet your needs following the assessment, then you will receive a full explanation of the support services which are to be provided and of any cost you or your dependent relative / friend may have to pay.
You have the right to receive a copy of your care plan which will confirm in writing
- your assessed needs
- the services we will provide to meet the needs
- the service objectives
- details of when you will receive these services and who will be responsible for providing the services
You will be asked to sign the care plan in order to indicate your agreement to the services offered. The care plan will also include the name and telephone number of the person responsible for your service from the department or any other agency / provider.
