Concessionary bus pass - application form

Use this form to apply for a concessionary bus pass. For more information about concessionary bus passes, including who is entitled to have a pass, follow the link in the left hand navigation.

To apply you will need to:

  • print off and fill in the application form at the bottom of this page. Paper copies of the forms are also available at the council cash offices and local libraries.

Please enclose:

Proof of eligibility required - (copies only please, for postal applications).

  • a recent passport photograph (please write your name on the back)
  • payment (replacement card only- please see the attached form for current cost)
  • proof of age such as a birth certificate, or retirement pension book
  • proof of residency on Anglesey

Additional proof of eligibility is required for the following types of pass:

Disabled:

Proof of eligibility required - (copies only please, for postal applications).

Doctor or specialist's letter, or proof of receipt of higher rate mobility component of DLA, or proof of registration of disability

Disabled with Carer:

Proof of eligibility required - (copies only please, for postal applications).

This pass is available to disabled persons who meet the above criteria and are so severely disabled that it would be impossible for them to use public transport without the assistance of a carer. (In this case no separate pass is issued, but the disabled person's card will be marked appropriately to extend the free travel to one person accompanying the disabled person.

Payment:

There will be no charge for issuing the pass, although the council will make a charge for replacing a lost pass.

How to submit your application:

  • in person at the Highways, Transportation and Property reception desk 
  • in person at the cash offices in Llangefni, Holyhead and Beaumaris 
  • by post to the Highways and Transportation Service
