The Social Services reception desk is open from 8:45am to 5:05pm Monday to Thursday. Between 8:45am and 5:00pm on Friday (except public holidays).

Anglesey Social Services Hospital Work Team - Ysbyty Gwynedd, Bangor
Anyone receiving hospital treatment, members of their families or carers can ask a member of the nursing staff or other hospital staff to make a referral on their behalf to the Hospital Social Work Team. Personal callers can make appointments by visiting or telephoning the Hospital Social Work Reception office at Ysbyty Gwynedd.

Carers
If you are a Carer, you have the right to request an assessment of your own needs and to receive services in your own right under the Carers (Services and Recognition) Act 1995 and the Carers and Disabled Children's Act 2000.
This can be done by contacting:
- The Housing and Social Services Department.
- If your dependent relative or friend is in Hospital, you can request an assessment through the nursing staff on the Ward or by contacting the Social Work Team at Ysbyty Gwynedd on 01248 384968.
- If your dependent relative or friend is receiving support through the Community Mental Health Team, you can contact Hafod Las on 01248 750191 or Craig Hyfryd on 01407 764231
- You can also request an assessment through your local surgery.

Mental Health Services
The Ynys Mon Community Mental Health Team is a multidisciplinary service that offers a range of therapeutic and supportive interventions for people with mental health related complaints.
It is based in two offices, Craig Hyfryd in Holyhead and Hafod Las in Llangefni, Coedlys a Support Bed Unit in Llangefni and St David’s Priory, Holyhead, where mental health support services and the Community Support Workers are based.
The team comprises Community Psychiatric Nurses, Social Workers, Occupational Therapists, Community Support Workers, Psychologists, Psychiatrists, Counsellors and administrative staff.
Access to a service is usually made by way of referral to the Duty Officer, based in Craig Hyfryd, Holyhead. This will often be by a GP or other health professional, or by self-referral. Family members or friends can also contact the Duty Officer for informal advice regarding a concern involving someone else but the knowledge and consent of the referred person would normally be required.
There is no ‘drop-in’ service at either office.
Craig Hyfryd and Hafod Las are open during normal office hours – Monday to Friday, 09-00 to 17-00. Outside these times, at weekends and Bank Holidays, the Out-Of-Hours is available on 01286 675502.
