Community mental health team

We are:

We are a community mental health team working from two bases serving the people of Anglesey. We offer specialist help for mental and emotional health problems to people and their families. This help might take the form of individual counselling or group activity. You may be seen by any one of a number of different professionals - clinical psychlogist, community psychiatric nurse, psychiatrist, social worker, occupational therapist, support worker - all of whom specialise in mental health problems. This support may take place at Hafod Las, Craig Hyfryd, or in your own home.

All team members respect and care sensitive to issues of confidentiality of their clients’ personal circumstances. Advice on aspects of care is sought from their professional colleagues. Information is recorded in the form of notes and on computer systems. There are statutory rights of access to these records by clients.

User Involvement:

Service users participate in decisions regarding their care and are represented on several managerial and policy making groups.

The Priory in Holyhead is a multi-agency resource centre servicing all of Anglesey - the emphasis is on joint working. The support workers work from the Priory and may be involved in individual work or running the social clubs which are available at Amlwch, Llangefni and Menai Bridge. For more information contact Lorna Thomas at the Priory, Holyhead Tel: 01407 761611

Complaints Procedure:

Complaints procedures are operated by both the Community Health Trust and Social Services. Information regarding this is available on request from the C.M.H.T premises.

The team has a responsibility for the training of professionals. We may request that students be involved with your care. If you have concerns, please discuss with the community team member.

Accessing the Service:

This can happen in a number of ways but for most people it would be as a result of contact with your G.P or members of the Primary Health Care Team (e.g health visitor, district nurses). You can also access the service yourself by contacting the Duty Officer.

A Duty Offuicer is available at:

Hafod Las - Tuesday, Thursday, Friday

Craig Hyfryd - Monday, Wednesday

during working hours.

It is possible that as your first contact with the service you may be offered a duty appointment. This is an opportunity to explore with the duty officer the best way for your needs to be met.

A number of groups run periodically throughout the year which may include:

  • relaxation and stress management
  • assertiveness

For details of other groups that may be running, lease contact C.M.H.T bases.
